FSA Benefit Bank FSA offers a monthly sum to each benefit-eligible employee to assist with the cost of benefit purchase. The benefit bank can be applied to the purchase of medical, dental, vision, supplemental insurance and cafeteria benefit plans.
Medical Plans FSA is partnered with five different medical providers that are consisted of both HMO plans and PPO plans. Each medical provider offers multiple plans to better meet your specific needs. The provider list consists of: Blue Shield, Chinese Community Health, Health Net, Kaiser, and Pacific Care.
Dental Plans FSA offers three different dental providers (consisting of both MHO and PPO plans), Delta Dental, PMI Delta Care, and Safeguard Dental. Each provider offers two competitive plans for you to choose from.
Vision Plans FSA offers three different vision providers, Medical Eye Services, Safeguard Vision Image, Vision Service Plan. Each provider offers two competitive plans for you to choose from.
Acupuncture/Chiropractic Plan FSA offers an acupuncture and chiropractic plan administered by American Specialty Health. This plan covers for thirty visits for each procedure.
Cafeteria Plan FSA offers Flex Choice 125 plan administered by Pacific Benefit Consultants Inc. This plan helps you to save money on insurance contribution costs, out-of-pocket medical, dental, or vision expenses, prescription or over-the-counter/non-prescriptive drugs, dependent adult or childcare.
Transportation Reimbursement - FSA offers a pre-tax transportation reimbursement plan administered by Pacific Benefit Consultant Inc. This plan helps you save money on costs incurred by purchasing transit passes, vanpooling expenses and parking.
Supplemental Insurance FSA partnered with Colonial supplemental Insurance offers you the opportunity to purchase additional insurance. Employees can purchase insurance that will cover disability, life, accidents, cancer, critical illness, and hospital confinement.
403(b) Tax-Sheltered Annuity Plan FSA partnered with AXA Equitable offered a voluntary savings program that supplements your future Social Security benefits and your personal savings or investment programs.
Employee Paid Time Off:
Personal Business Leave (PBL) FSA employees begin to accrue time off effective first day of employment. Employees accrue up to three days of personal business leave days. Once three (3) PBL days are accrued by an employee, no more shall accrue until a portion are used, at which time an employee may again accrue PBL days up to the three (3) day maximum. Any unused time will rollover into the next fiscal year. The accrual amount varies depending on employee status and FTE percentage.
Vacation FSA employees begin to accrue time off effective first day of employment. Employees accrue up to four weeks of vacation (21 days). Once twenty-one (21) days are accrued by an employee, no more shall accrue until a portion are used, after which vacation leave shall again begin to accrue up to the twenty-one (21) day maximum. Any unused time will rollover into the next fiscal year. The accrual amount varies depending on employee status and FTE percentage.
Sick Leave FSA employees begin to accrue time off effective first day of employment. Sick leave shall accumulate to eligible employees, and may accumulate up to a maximum of four hundred fifty (450) hours. Employees may not accumulate any additional sick leave until their bank of 450 hours has been reduced. Any unused time will rollover into the next fiscal year. The accrual amount varies depending on employee status and FTE percentage.
Paid Holidays FSA observes twelve holidays annually, during which times Agency staff will receive time off with pay. The pay will be based on employment status of part-time or full-time. Actual holidays will vary based upon program needs and the feasibility of closing the program for the day. These holidays will typically include:
